Title

The effect of antidepressant treatment on N-acetyl aspartate levels of medial frontal cortex in drug-free depressed patients

Overall design

The present study was performed to determine whether there was any effect of antidepressant treatment on the metabolite levels in the left medial frontal cortex as measured by proton magnetic resonance spectroscopy in depressed patients. Patients diagnosed as having major depressive disorder according to DSM-IV (depression group, n = 20) and healthy volunteer subjects (control group, n = 18) were included in the study. Four patients were treated with fluoxetine 20 mg/day, three patients were treated with paroxetine 30 mg/day, and the rest of the patients (n = 13) were treated with venlafaxine (mean: 170 mg/day). The remission was observed in 70% (n = 14) of the patients. Other patients' HAM-D scores decreased more than 50% before the second MRS scanning. The 1H-MRS scanning was performed when the depressed patients were free of antidepressants (baseline) and after the response to antidepressant treatment (more than 50% decrease in HAM-D scores) has been observed for more than 1 week to eliminate the risk of placebo effect and temporary well-beings. Single voxel, 8 cm3, 1H MR spectra of left medial frontal cortex was acquired both before and following antidepressant treatment. The concentrations and ratios of N-acetyl aspartate (NAA), Creatine + Phosphocreatine (Cr + PCr) and Choline (Cho) were measured. Metabolite quantification was reported relative to total creatine as the reference peak.
