Title | Multinuclear magnetic resonance spectroscopy studies of brain purines in major depression |
Overall design | This study was undertaken to determine whether proton magnetic resonance spectroscopy (1H MRS) measures of the low-field purine resonance, which arises primarily from adenosine phosphates, can be used to assess abnormalities in cerebral purine metabolism in depressed adults. Data from 1H MRS and phosphorus-31 (31P) MRS were acquired for depressed (depression group, n = 38) and nondepressed comparison subjects (control group, n = 22). Intensities of the purine resonance, by 1H MRS, and of NTP, by 31P MRS, were determined. The depressed subgroup participated in an antidepressant trial consisting of 8 weeks of fluoxetine 20 mg/day treatment. Hamilton depression scale scores were determined at baseline and at the 8-week endpoint for all depressed subjects. The depressed group was separated into responder and nonresponder subgroups on the basis of the Hamilton depression scale scores. The criteria for responder status were determined a priori as achievement of a 50% or greater reduction in symptom severity on the Hamilton depression scale and a post-trial Hamilton depression scale score of 7 or less. Of the 38 depressed subjects, 17 met the responder criteria (responder group), and the other 21 were designated nonresponders (nonresponder group). For 1H MRS, metabolite quantification was reported relative to total creatine as the reference peak. For 31P MRS, the peak area of each metabolite was normalized by the total phosphorous signal. |
